Adding custom script in a chatbot message

I would like to add custom script in the chat where I could allow users to register for my application. Right now my application has a registration tool (form) and I would like to add this form to my chatbot. Alt I would like to create a form and have the details of that form sent to my application.

Where can I find the documentation on how to conduct this opperation.
